Verify Your Benefits

Even when a health plan is accepted, individual coverage can vary. Before your visit, we recommend checking your benefits for primary care services, including your copay, deductible, referral requirements, and any services that may require prior authorization. Our office can help provide the information your insurance company may need to verify coverage.

What to Bring to Your Visit

Please bring your current insurance card and a valid photo ID to each appointment. New patients should also bring all medications including OTC, information about any specialists involved in their care, and any referral or authorization documents required by their health plan.

If you are following up after an emergency room visit, hospitalization, surgery, or specialist visit, please bring any discharge instructions, hospital paperwork, test results, medication changes, or other records you were given. Having this information available can help Dr. Esquivel better understand your recent care and determine the appropriate next steps.

Self-Pay & Payment Quesitons

Patients who do not have insurance or who choose to pay directly for certain services are welcome to contact the clinic for current self-pay information. Fees may vary depending on the type of visit, testing, or service provided. If you have questions about payment, please call the office before your appointment so our team can help you understand what to expect.
